About

Moving On Asthma has been developed by the Respiratory Research Team at Sheffield Children’s NHS Foundation Trust.

As a team, we are passionate about improving outcomes for children and young people with asthma and this resource is designed to both support clinicians in adolescent health and equip young people with self-management skills as they prepare for the move to adult care.  Moving On Asthma does this by addressing the goals of the Ready, Steady, Go Transition Programme.  All content on the website is evidence based and in line with current national guidelines.

We would like to thank all the young people and clinicians who have been involved in the design and creation of this resource.  We could not have done it without you!  A special thanks to Lynn Rishworth and Rex Domino for making our dream of an educational rap become a reality and finally, we would like to acknowledge all the help we have received from our collaborators:
